Tips about your upcoming appointment
Please bring your prescription from your doctor, your insurance card, any relevant x-ray/MRI/CT scan reports and a list of your current medications (over the counter and supplements too) with dosages.
Dress comfortably so we may access the area that needs to be treated. For instance, shorts for a leg injury or a tank top for a shoulder injury. We do provide gowns for your convenience.
For your convenience you may download the above forms to prepare for your visit, or alternatively, please arrive 15 minutes before your appointment time to complete paperwork.
Initial visits generally last 1 hour.
